Choice and Support
Anglicare at Home understands that maintaining your independence is important. We provide a wide range of services aimed at giving you choice and flexibility in the way that care and support is provided to you so you can remain living at home for as long as possible.
Individualised Care Plans - Consumer Directed Care (CDC)
As everyone’s needs are different, an ANGLICARE Care Co-ordinator will arrange a time and place suitable to you, and your friends and family if desired, to discuss your situation and needs.
Next we will find a team member who has the right skills and the right approach and balance to fit with your lifestyle, spiritual and care needs. Together we will develop and manage your care plan which outlines the care and services you would like within the amount of the funding available. We also provide ongoing support to adjust your plan if your needs and circumstances change.
What packages are available?
Anglicare at Home offers a range of services from daily personal care, household activities, transport and social and recreational support to specialised nursing care. In addition to these general services, we also offer supplement services to those with dementia and their carers.
Home Care packages available are:
Level 1 For basic care needs (similar to Home and Community Care Services – HACC)
Level 2 For low-level care needs (the former Community Aged Care Packages CACP)
Level 3 For intermediate care needs
Level 4 For high-level care needs (the former Extended Aged Care at Home EACH and Extended Aged Care at Home Dementia packages EACHD)
Eligibility
1. Assessment
The first step to receive a government-funded home care package requires you to be assessed by an Aged Care Assessment Team (ACAT).
This free assessment determines your eligibility and the level of funding provided to you to receive government-subsidised home care services.
To arrange your assessment you can contact your doctor or local hospital who can refer you to an ACAT for an assessment. Or, you may like to contact your local ACAT directly to arrange your own assessment.
2. Contact ANGLICARE
Once your eligibility is approved, contact Anglicare at Home. We will meet at a time and place convenient for you, assess your needs and work in consultation with you, and your friends and family, to develop a plan tailored to your needs and requests.
If packages are available, we can start you on the program immediately. For areas with a waiting list, will we work with you to make alternative arrangements so you receive the services you require until a package is available. Once a package is available we will arrange that for you.
Costs for Home Care packages
The Australian Government pays for the bulk of aged care in Australia and determines the basic fee of home care packages.
The basic fee for a Home Care Package, including Consumer Directed Care, that a service provider may charge is up to $131.46 per person, per fortnight (from 20 September 2013 to 19 March 2014). This rate increases on 20 March and September each year in line with changes to the Age Pension. This applies per person receiving a Home Care Package, even if you are a member of a couple.
People on incomes higher than the Age Pension, may be asked to pay extra.
ANGLICARE will not deny you the required care because you can't afford it. Please contact one of our Care Coordinators to discuss.

Staff
Our reliable HomeCare team offer support, understanding and years of expertise. Each carer is committed to providing quality in- and out-of-home care services.
The Anglicare at Home team consists of Registered Nurses, Physiotherapists, Chaplains and carers.
Most of our carers have a Certificate III and receive training and support regularly. All staff receive induction and ongoing mandatory training.
Carer Support Groups
Anglicare at Home has developed support groups to provide the opportunity for carers and loved ones to talk with others who are in a similar situation. These groups are run by an ANGLICARE Care Coordinator and they enable you to share experiences, learn from others and have your questions answered.
